Some approach procedures are titled, ex: "ILS or Localizer 23R" and some are just titled, ex: "ILS 23R". The title that only says "ILS" still has localizer only minimums. What's the difference????

If the approach says ILS or Localizer then you have the option to shoot the approach as either a precision (ILS) or a non-precision (Localizer) approach. An ILS approach has LOC only mins so that if the glide slope is out of service an approach can still be made, just to higher mins.
