Default Safety Quizz - Forsee the accident

I used to hold group 3 commercial aviation maintenance licenses. A dangerous situation may arise as a result of the interaction between the many professionals who work on an aircraft.

A flight engineer might leave the APU running in a 747 when s/he departs on aircraft if ground power is not available. An avionics engineer might carry out checks on pneumatic valves. An engine person might carry out inspections on a thrust reverser.

What is the accident that might result?

Please do not hesitate to contact me if you do not know the answer?

It would be great to hear other novel situations that could lead to an accident.

How about leaving gloves and tools inside a fuel tank during routine maintenance? Then, having those foreign items clog the fuel lines to the engines? It has happened.
