If a failed, meaning Q3,check-ride is not listed on your AF Form 942; which is the left hand side of your Flight Evaluation Folder then you have never failed an Air Force check ride. I went to pilot training a long time ago so maybe things have changed but my AF Form 942 when I retired in 2012 just showed a single entry for all of UPT and that was a Q1 qualified T-38 check ride at the end of the program.
If you don't have the 942 showing the fail and the AF Form 8 check ride form write-up then I think you can honestly answer the question about the check rides by saying you have never failed one -- assuming you kept a clean record after being awarded your Wings.

By "downgraded", I believe that they mean, downgraded in position. Like if you were an aircraft commander that got downgraded back to a co-pilot. In any case, I don't believe that you need to put that on your app. I don't think that a Q1- is what they're looking for in that block.

OK, then there's the "Commander Directed Downgrade (Q3)" because your buddy did something dumb (overspeed by 3 knots in the blink of an eye) when you were the Pilot Not Flying IP and you didn't catch it fast enough to take the airplane. Is that a busted checkride? Answer: no. Did I put it on my app? Answer: yes...just in case it comes up when they look at my 942 which now shows a single Q3. "And here is what I learned from it, blah, blah, blah..."

Having said that, I was at an Emerald Coast prep very recently and when I told the instructor I had put on my addendum that I failed a T-37 UPT check, he expressed some concern that it might push me down a little in the application scoring pile. However, he also said it will not keep me from getting an interview. Once you are in the interview, the company has accepted it and no harm no foul. But, if they get even a small hint you've been less than forthright, they are going to probe. If they feel like you have been deceptive, your done.

I asked this question at Vets in Blue (JetBlue) and they said they want to know about failed UPT checkrides. I also disclosed this information at SkyWest, CitationShares, and NetJets and got hired at all three.

What he said. They don't care so much that you busted a ride, just that your honest, own up to it and develop a lesson learned.

I've been back and forth on it, but in the end I'm more comfortable disclosing it. I think Southwest specifies not to include UPT stage checks, but for the rest of them I mark it and provide an explanation. With all of the hiring going on I think there's more to lose from not disclosing it than there is from putting something that happened over a decade ago on the app with plenty of experience to overcome the slight black mark.
Just have a good answer prepared for the interview about how it was actually your form partner's fault or your instructor's fault or whatever. (joking of course!)
