Default FAA medical (Class 3) - foreign AME

When getting a Class 3 medical from an AME, do you have to specifically request a student pilot licence? Or is the medical certificate the licence?

Also, if I get my medical from an FME outside the US, will there be any issues when I start my training there?

Yes, tell him you need the student pilot cert. It is a combined third class/student pilot cert but the normal third class is just a medical.

If he is an FAA AME, it will not matter where he is located. If he is a foreign AME who is not certified by the US FAA he cannot issue a US medical.
