I don't have the book handy but it should be in part 61.
You have to ground yourself if you are too ill to safely fly. Generally you can unground yourself when you feel better if it's a routine illlness (cold, flu, etc). Significant illness or injury requiring medical care would generally require a trip to the AME to get cleared to fly.

But if in doubt, I think the PIC should throw the flag in the event of a sick crewmember (I would). That is certainly with PIC discretion.

Disregard F/As at your own peril. See: Sullenberger. His ladies got them all out safely. He gets all the credit but they did their job too when it counted.

I've had 2 FA's removed from flights since I thought their illness would impair their ability to safely perform their job or cause a bigger problem. Didn't have to do a carpet dance. As the CA you are responsible for their safety/ability to perform their duties also....
