Another good accident read. Not a good outcome for the pilot and passengers obviously, but the accident investigation is fascinating IMO. They were able to ascertain that a critical bolt was not attached at the time of the accident, inferring that it came loose and flew off, which caused the loss of control and subsequent crash. The investigation goes in depth with the maintenance interviews and materials analysis.
