I had that USAir Captain as a JSer five years ago. He was one of the most professional pilots I'd ever met and was surprised to find out he was involved in that incident. I think he got his job back after many months of investigation as it was found to be an accident and not his being careless. I'm sure someone on here has the definitive answer on this and hope they can chime in....
Yeah it was the the locking holster that the boys put out for a while. And it wasn't the only accidental discharged that was caused by it. We told them it was going to happen and in typical government fashion it was ignored.
