I disclosed it on applications (better safe than sorry). I do have the packet from the FAA and it doesn't have anything about it. Somewhere I believe that when employment verifications happen the company can disclose any failures I have had that are not on the PRIA. That was my question also. I've ended up just contacting my old airline and getting the answers I want there. I just don't want any surprises when I interview. As the previous person said, just disclose it and make it positive because you had to have learned something from it.
