There are plenty of guys that FX has both interviewed and hired with much more substantial discipline on file from their previous employers.

The application (at least before the move to the current website, it was Question #20 for those following along at home) used to ask:
"Have you ever received formal discipline from your employer?"
Seems pretty clear-cut to me.
There were other questions that addressed accidents and incidents, formerly questions #13 and #14.
I don't see how the guy quoted in the original post could have had uncertainty about how to answer that question.
