The coup de grace was a CFII unhooking his seatbelt, rolling his seat back, and going under the panel to retighten a known faulty cannon plug on one of the CDIs while the trainee - still in foggles - held the aircraft straight and level but heading for rising terrain.

The terrain gradually rose to the level of the aircraft while the trainee dutifully continued to hold it straight and level. The trainee survived, albeit with some serious injuries. The CFII, sadly, did not.

I actually had one of my student flights in that aircraft (a PA-28) maybe half a decade earlier although I knew neither of the people involved and was off in college by then.
