Been exchanging with a couple of friends at EK, they used to have a Takeoff Review procedure that of course covered the FMA and MCP settings but it was taken off the procedures by an Assistant CP with the argument that this is something professional aviators do and there is no need for a reminder.
Also about the two additional pilots, they were telling me that more times than not, the folks on the jumpseat pay no attention to the procedures of the flying crew and that they are often on their phones until the aircraft is about to take the runway.
I would suggest that perhaps there are lessons to be learned from this because I must admit that the behavior of jumpseat occupants that they described is something that I have seen.

With 4 pairs of eyes this shouldn't have been missed.
