I've seen multiple deviations of all kinds and a few factors seem to play into whether you hear something or not: just one of those excitable/angry controllers; you scared him/her; you didn't seem to recognize/react to the problem. If they think you'll repeat offend or you don't understand the importance, they're more likely to take it further. Oh, and if his/her supervisor is standing over the shoulder.
