Does Southwest have life insurance and disability insurance? If so, is it a good deal?

Yes, yes, and no...

The issue with company provided life insurance is that coverage ends if you go out on medical and the company terminates your benefits. Once your sick time is gone you are no longer an employee. You can use your imagination on how that could screw you over big time. If you need life insurance you should buy it outside of Southwest.

As far as disability insurance, the company paid plan is lacking so most pilots have extra STD/LTD insurance. I'm well covered but I pay for most of that coverage myself. I take solace in that our medical premiums are so much cheaper then the legacies, but this is an area that hopefully will be addressed in our next contract.

Plans are arranged by SWAPA and payroll deducted.

I believe the actual provider is MetLife.
