I pay $233.00 per paycheck for a family of 4 under the Cigna Open Access Plus In Network Plan. So far no complaints here; pretty much everyone takes Cigna hence it isn’t hard to find in network providers. I believe when I was hired, there was a 2 week waiting period.

Another note on planning for training pay: while you will receive per diem starting from day 1 of training, due to the way that our pay is issued, you won't start seeing the per diem in your paycheck until the end of the 2nd month.
We are paid on the 15th and the last day of each month. The pay on the 15th is 1/2 your guarantee, and the pay at the end of the month is 1/2 guarantee plus any additional pay (but, this is for the month prior. So, your July 31 paycheck is 1/2 guarantee plus whatever was over guarantee for the month of June, be that per diem, or rig pay above guarantee.)
So, for your first month of training, the only income you should expect is the $1,600.
Negative, you will be considered the junior base day 1 of training. They tax. percentage of per diem here while in training. Different from what I’ve experience when I was at my regional.
The IRS has standard per diem rates different from the GSA/DOD/State Department rates. And the tax write off of the difference between what you were paid and the GSA et al. rates went away with tax reform a few years ago. The IRS rates for transportation workers are $66 a day domestically and $71 internationally. Atlas can’t even figure that out so they just withhold on what you’re paid above the $71 a day/$2.91 an hour rate even if you’re domestic.
