Do both pilots have to be RVSM qualified for a crew to be RVSM?

Yes. Ever 12 months for 135; 24 months for 91.

Agree with the first part but not the second. There is no blanket requirement for recurrent RVSM training. Individual RVSM approvals may be based on some sort of training stipulation at the FSDO's discretion however, so make sure to check your manuals

I think our training in RVSM was two lines in a bulletin.

It is the same at American but I need something a little more substantial to offer the FAA for an RVSM LOA.

Our rvsm LoA requires no recurrent training that I know of.

Read the ops spec B046 that goes along with the LOA. It contains text which states who the RVSM training is provided by. King Schools is a very common vendor. There is no recurrent training requirement for Part 91 ops.

Not necessarily true. Everything depends on how your RVSM manual is written and the details of your LOA.

Read Appendix G to 14 CFR Part 91, Section 3. For Part 91 operations, there is no regulatory requirement for recurrent RVSM trng.
