Stage Check Failure Location
 
Greetings All,

I have a Part 141 stage check failure (not checkride). For the Delta application, I included it under the "Have you ever failed ANY checkrides (military or civilian) including phase checks, proficiency checks, FAA check rides, or line checks?" question.
I've been told to include it under the "Have you ever been the subject of an evaluation board or been removed from flight status, downgraded, or failed any portion of flight training either civilian or military?"

Which one would you include it under? Thoughts? .....does it matter as long as it's included?

Semantics but a 'stage check' is not a 'check-ride' so mention it under option #2.
Which stage check by the way? End-of-course or intermediate?

Option # 1 is for any checks failed while employed as a 135/121 pilot or obviously in the Military.
These all have mandatory checks.
Proficiency checks, currency checks and that sort of thing.
