PRIA question
 

I have an interview coming up;

Does anyone know if I need to fill out the PRIA form "8060-11" (release of records by previous employer) as a flight instructor in a part 61 flight school?

Also, in the instructions given by the airline, they say to, fill out completely all the required forms;
Should I fill in information like, my name and address etc., in the sections labeled, "to be completed by the air carrier"? (they are already partially filled out by the air carrier)

Lastly, should the forms be signed and dated beforehand, or wait till the interview?

Yes. EVERY job that you have flown and received a paycheck for you need to fill out a PRIA form. Whether or not the flight school sends any info back is kind of up to them.

If you have questions about how exactly to fill out the paperwork, call the HR contact that you have PRIOR to the interview. DON'T SHOW UP WITH INCOMPLETE PAPERWORK. At least when I was working for pilot recruitment at Atlantic Coast, that was pretty much a no-go/elimination item.

Yes, you need to complete a PRIA form even if you were a flight instructor. They keep different records than an airline, but that's not your problem - it's the airline's to sort out. Any flying job - any.

Careful about the "to be completed by carrier" section - are you sure it is YOUR address the form is asking for? It may be the address of the former employer or even the current prospective employer. Read it carefully and look at it logically - what is the form asking for? If you're unsure try scanning it and posting it here, let's take a look at it. You can also call the recruiter, or your point of contact, and ask them. Everything should be complete upon your arrival at the interview. Signed, dated, etc.

Ok, part I is the air carrier information - including the air carrier mailing address. You wouldn't know this information. THEY complete this part.

Part II is if YOU want a copy of what is being sent. (Your mailing address goes in part II) - I highly recommend you always request a copy of what is being furnished.

Have no idea why there is a date - it may be when this form expires or needs to be re-evaluated? Who knows....it's the gov't. ;)

Actually, according to the instructions the "to" section in part one is for previous employer address. I did contact HR, they told me to fill in anything that applies to me.

As far as who you need to request records from, per the overview and instructions/guidance regarding FAA Form 8060-11 Air Carrier and Other Records Request...

Hence it is NOT REQUIRED that these forms be submitted to Part 91 entities with whom you were previously employed.

Certain other exceptions apply to PRIA, for instance if you only conduct intrastate commerce and do not carry mail (as a company, not as an employee) PRIA is not-applicable.

I had thought it only applied to 121,135 carriers. But some say it applies to any pilot position.

I will put my flight school info in just to be safe, they can decide whether to send it or not.

I actually used to work for a 121 carrier, but not as a pilot; part-time ramp agent (Comair, now Regional Elite Airline Services).
