I am filling the FedEx application for pilot employment and one of the questions is

Have you ever received fomal discipline from your employer?

I had to attend focus training for a couple hours due to the fact that I conducted an improper ice test. In the airplane I was flying the checklist call for an ice test to be done while having the parking brake set. I did not set the parking brake and did it while on the taxi and holding the brakes.

Due to this reason I had to attend focus training which consisted in telling me to set the brakes next time.

My question is do I need to check yes for this question?

Is there a paper trail?? That should answer your question....

There will be a paper trail for retraining, but it is not necessarily disciplinary. Letter of reprimand would be disciplinary. Either way, list it. It would be worse to find out that they wanted that there later.
