What does this question mean?
 
11. Have you ever failed a Flight Check-ride, Proficiency Check, Flight Evaluation, or Upgrade attempt (Aircraft or Simulator)?

so filling out my SWA app on pilot credentials and it asks the above question.

are they referring to only part 121 rides? or any and all aviation related testing?
I am asking because I attended a part 141 school and have 3 EOC busts.

my CFII, CFI and comm-multi instrument oral exams.

does that count?

I put them down just in case but wanted a second opinion.

I am not a 121 guy but if you do a search using the google window at the upper left you can locate opinions from experts and I recall seeing this question more than once before. If I recall, end of course (EOC) 141 check rides are counted in the bust figure.

If it was a checkride or end of course stage examination that (would have) resulted in the issuance of a certificate, I would report it.

That is the conservative answer. I suspect SWA would be a lot more interested in your 121 training record than in your entry-level flight training.

In the case of a 141 oral exam they might never find out since there is no pink slip and no record in your logbook (I assume). But if they did find out you would probably be fired. The FAA is going to have to improve disclosure from 141 programs due to colgan...your school might have to cough up records if they still have them.

thanks, I do not want to hide anything and that is not my intention, just trying to digest the question and get some second opinions.

The question is poorly worded, i found myself re-reading it. My take is it includes all flight training including airline. Many ask this question including oral and written exams.
