Perhaps you should speak with an Aero Logic or Lufthansa Cargo union pilot. That is exactly what they do. Germans and Swiss love their schedules.

Regarding waiting for freight in CVG. DHL strategically figures out freight shifts when things are running late. The put the time sensitive items on the aircraft with the best chance to make the European sort. They add extra fuel for high speed burn and off it goes. I have done a number of these. Have other companies done these?

DHL owns Aerologic...it is a joint operation with Lufthansa. Big difference if they are not competing with everyone else. DHL can tell them to do whatever they want..
As far as extra gas and Hi speed cruise yes...did it all the time West to East when things are dicked up. Also make exceptions when K4 is routinely late ( usually up to 2 hrs because of maintenance according to our Mechanics in SEA ) on Mon am coming from LAX.
