All you need is a passport. I've done it several times, and I just did it two days ago. Take the "First Ferry" from the China Ferry terminal in Kowloon close to the Ocean Terminal...or there is also a high speed one on the HK island side by Central....cost 140HKD each way

The Lisboa has a good band, the Venetian is good if you like to shop (not for me thanks, I'll take a martini, instead).
A little pricy, bring your own booze.
wasn't impressed with the gambling, you could hear a pin drop in the casinos.
