when incidents such as these occur, does the Safety department send out an FYI or bulletin to the crews. lessons learned, or "this happened to our guys, FYI" type stuff

At my company they like to pretend that nothing happened and sweep it under the rug. Really could be used as a teachable moment for us all but sadly not all think this way.

If this did happen, teachable moment already as I am unfamiliar what “incorrect switch” directly means in this particular case? Incorrect runway/departure switch (box work), Heading bug (select) in left quadrant vs right and engaged vs autopilot switch, FMS acting squirrelly and we stare at it too long, executed something manually which leads to deviation? Could be a wide range of circumstances and I hope it is shared so I may add it to my bag of tricks.

Promise you, not quarterbacking - I have added a new statement to my vocabulary vs “and the monkey flips the switch” as I am not immune to mistakes and we do the best we can. We have had our own incidents (HKG included) and some of them were show cased in the sim to highlight these teaching moments and I am grateful for the transparency. It helps the community as a whole. Be safe out there and teach me OB1.

Runway change same SID different first fix, at 400’ airplane turns to the fix of the parallel?
My best guess.

We will know when the investigation is completed. I doubt any useful lessons will be learned on APC.
