has a CFI ever been Violated for a paperwork issue?

What do you think you did wrong? Let's start there.

Our FSDO made a CFI do a 709 ride for repeatedly sending CFI applicants to their checkrides with improper endorsements. The inspector telling me about the incident said the CFI was counseled first, repeated the errors again, and then was given the 709 ride.

At my school, we had a CFI canned for endorsing a few students a few lessons before the end of the 141 syllabus so he could go on vacation out of the country. I don't know if the FAA ever did get involved, but for something as blatant as this and it being a violation of 141, he was fired.

Yes. But most likely they will talk to you first, maybe give you a 709 if it's a gross departure.

This assumes honest mistakes, not falsification...in the latter case, they'll probably shred all of your tickets.

So has anybody ever heard anything about a CFI getting a violation for making a paperwork error on an IACRA? as in signing it too early for a retest?
On another note what are FAA inspectors looking for when they ask to review your logbooks and Instructor records?

It all depends on what you do wrong.. I would say that most FSDOs would at least try to work with you if it's the first time you mess up some paperwork or anything, but you never want to mess up with the wrong person
