Immediate family (spouse, children, parents) fly free unlimited on all Frontier flights. All of the above have access to ZED fares on most airlines (notable exceptions are UAL and DAL).

There are multiple options for health insurance. I believe all are PPO. There’s a High Deductible plan with HSA option, a traditional plan with 80/20 coverage, and then the “legacy” plan that has high coverage and extremely high premiums. Most do the HSA or traditional plan. Personally I prefer the HSA.

At JB, coverage begins the first after your first full calendar month, so if you started April 11th, coverage wouldn’t begin until June 1st.
