You can apply online for a form FCC 605-FRC

I'm not sure about the cost I got mine a couple of years ago. Pilots need these in Canada and other countries outside the U.S.

If you go outside the US you'll need a passport too if you don't already have one.

I think there's a way where you can get it for free if you fill out the papers online or something. Can't remember.
