Flight Safety Crew Tags

If you do your training through Flight Safety, here's a link to get new crew tags. Just fill out the form and they'll mail them to you, up to 4. Pictures as well of your aircraft if you want it..

Why does it link to Flight Safety Survey? Shouldn't it go to a flightsafety/whatever?
