Restricted Radiotelephone Operator Permit
 
I just put the required forms for the Restricted Radiotelephone Operator Permit in the downloads section. It's been years since I applied for mine, although the process isn't getting any easier by the look of things - hope the downloads help.

Be sure to download all three forms:

Form 605 - Operator Permit application
Form 605C - Aircraft
Form 159 - Payment processing attachment

do it online it's much easier you could call a 800 number as well for help.

Marginally, go figure the FCC has one of the most jumbled web sites :
