R.I.P. Very sad to see so many young lives needlessly lost.
Many things were very wrong with the operation, hopefully changes are incorporated into operations and attitudes to prevent this type of accident in the future. |

My assumption when I read his post was that he was referring to the Check Airman that was the Captain on this flight, assuming this was truly her first flight in the airplane as reported, who obviously showed horrible judgement.

4 hr+ flight with 81 pax and bags, even full fuel for the Avro would have put them at a precarious situation upon arrival at the destination even if things were working out great. A simple 15 minute delay for another aircraft put this aircraft in a flameout situation. The investigation should reveal all. Did the FO have the training (CRM) and experience to call out to the CA ahead of time that she was not comfortable with the fuel state? And who was PF? The CA seemed to be the one doing all the talking on the radio, so if the FO was PF that should be okay, but if it turns out the CA was also the PF, this is going to be another CRM disaster accident. |
There were two PIC's listed and one SIC. I'm sure the investigation will sort out who was PF/PM.
