Default Documenting checkride failures

Hey everyone.

I'm getting my information together on the application site, and it says to document your checkride failures in a separate document and attach it. I don't see much in the way of instructions, so I thought I'd ask here. I searched the forum but didn't see any threads that addressed this specifically. My apologies if this has already been asked.

How much detail should I give in describing the failures? Do they just want date, place and type of checkride or should I go into more detail?

I'd treat it as a TMAAT event, lay out the facts of the failure, and how you overcame it.
