Default Who has pillows and blankets?

With all the media attention on JetBlue's new pillow/blanket combo for $7, I was wondering which airlines still have pillows/blankets on board? (I mostly commute on JetBlue and UsAirways, so i may be out of touch). And are they on Mainline vs Regional/ domestic vs international/ many vs just a hand full?
