PRIA forms for military?
 
How are military guys supposed to fill these forms out? The 11 and 12 are particularly perplexing since any drug use would have led to being kicked out, and how do you fill out the contact info for previous employer?

Contact for last employer I have always used my last command.
I got a PRIA from the FAA for the FAA.
I'd have to look again to see what it said.
Concur about the drug use.

Go to the pilot credentials website, select Hiring Forms--paperwork instructions

Go to page 2 and look at the last line. It tells us military folks to fill out the PRIA paperwork anyway.

What day do you interview? I'm heading out Wed for a Thurs 0800 interview. Just wondering if we'll cross paths.

Is this just for folks going to interviews? Or did I miss something on the pilot credentials site (I've got PC apps in for FDX & SWA). I'm on the road right now with a goofy schedule and limited access to real/full internet...

The company that i'm interviewing w/ told me to fill out the 10 and 10A...

....they said military was exempt from the 11, 11A and 12.

...also did NOT have to notarize the NDR form, but had to fill out/sign in blue ink.

Grumble -

I'm not sure what forms I signed or didn't sign, but my PRIA has a control number on it and lists my medical certificate and airman certificate(s), and summaries of legal enforcement actions resulting in a finding of a violation, which may include violations on current and previous certificate numbers

It mentions that pending legal actions might not appear until the case is closed and that recent additions to an airman's certificate that were issued within the last 6-8 weeks or less may not have made it through the system yet will not appear.
