OPR = "Officer Performance Report" (USAF-speak); I'm sure what you describe is the same thing (in USA-speak). I don't recall specific wording, but I'd be willing to bet the instructions that asked for the OPRs actually said something along the lines of "military performance reports, personnel records, etc." to cover all bases....

Make sure to have a paper copies of the following:

-DOD IAA CyberAwareness Challenge V2.0 certificate (within the last year but not going to expire in the next 3 months)
-Security Administration (formerly Information Protection) certificate
-Human Relations certificate
-Force Protection certificate
-Suicide Prevention certificate
-Pre-deployment Sexual Assault Prevention Reporting certificate
-CBRN Defense Awareness certificate
-SAPR Annual Training certificate
-Self Aid Buddy Care certificate and hands-on training certificate
-SERE 100.1 certificate
-Law of Armed Conflict certificate

I thought only the army did all that dumb stuff. Guess not.

The Officer Evaluation Report (OER), which is your annual or PCS report. When I started in the AF ('85), we used an OER. Later, someone made Field Grade by suggesting we change it to OPR, because we shouldn't be too much like the Army, you know.

And this for the AF types: for Pete's sake, it's "Forms 8", not "Form 8s". Insert emoticon of your choice in the following space: ____.
