Default (NON) Promotion and ROPMA?

I have recently run into a few friends, all of them are close to my year group or from my AMS class, and they are all still Captains. For reference, I've been a Major for 2 years. It seems that their states or unit leadership are feeding them lines about control grade issues. I have heard of control grade issues for LTC, but never Major. It seems some squadrons have just plain pushed their people to ROPMA for Major as a matter of policy? Every squadron pilot is in an O-4 billet, so what does leadership gain by doing this? What possible reason could leadership have to keep people, with clean records and PME complete, from being promoted? Leadership wonders why people are bailing left and right, then does this? I guess they don't realize THEY are part of the problem!

Bad leadership? What am I missing?
