Is the pilot weight restriction mainly to stay under max gross or stay in the envelope?

Default Medical weight VS Passenger Weight

We use the LifePort systems in our King Air 200's. In medical configuration the aircraft are about 200 pounds heavier than in passenger configuration.
I'm looking for pilots if your interested in Lafayette, LA let me know
