OnLine Form 8500

Default OnLine Form 8500

When filling out the online 8500, when getting to medical history, just how far back are we required to disclose items. Had lasik surgery more then 8 years ago, the faa knows about it from years past, do I still have to keep putting on the form? Also, do we have to put in our visits to our AME for our medicals? Last AME I saw said it was not needed. Thanks for anyone taking their time to reply.

When you log onto MedEx and start a new application, there's a help link that includes instructions and explanations for each block. Here's what the instructions say about doctor visits:

VISITS TO HEALTH PROFESSIONAL WITHIN LAST 3 YEARS List all visits in the last 3 years to a physician, physician assistant, nurse practitioner, psychologist, clinical social worker, or substance abuse specialist for treatment, examination, or medical/mental evaluation. List visits for counseling only if related to a personal substance abuse or psychiatric condition. Enter the date of visit as month and year (e.g., 01/1950), name, address, and type of health professional consulted and briefly state reason for consultation. Click the "Add" button. The visit information will display below the appropriate column headings. Repeat this process to add all relevant visits to medical professionals in the past 3 years. Each visit's information can be updated or deleted by clicking on the "Edit" or "Delete" link displayed to the right of each visit listed. Multiple visits to one health professional for the same condition may be aggregated on one line. Routine dental, eye, and FAA periodic medical examinations and consultations with your employer-sponsored employee assistance program (EAP) may be excluded unless the consultations were for substance abuse or unless the consultations resulted in referral for psychiatric evaluation or treatment.

It looks like the last sentence confirms what you've heard about FAA exams. Also, when you did Lasik, did you fill out a 8500-7 if you were flying at the time?

Three years, and technically you're supposed to keep repeating previously reported visits with each medical until they drop off the three-year horizon.

My AME said that was so that if you went to a new AME he would have visibility on the history.

O.K. thanks, that is the impression I got after talking with my old AME.

Others have answered the question about visits to healthcare professionals, however you also asked how far back do you go in medical history. The answer is birth. "Have you EVER..."

For previously reported items with no changes, you simply state that, i.e. "Previously reported - no changes"
