Most AMEs will say just do your best to remember what you've put. Big thing is that you've reported it in the past, so it's not like you're trying to hide something. You could always check your insurance records for dates of service if you needed. Or ballpark a date and tell your AME and see what he says. Also, if you're really hung up about it, get your medical records from Airman Medical Certification Branch. For a fee they will send you everything that's ever been submitted to the FAA including all your medical applications.
