Default Short Research Survey for all pilots

Survey for all pilots employed by United States:

https://goo.gl/forms/HwMtXPlj

Hello,
I am Noor Said, and I am conducting a research project regarding the influence of FAA regulations on development of kidney stones in American airline pilots. In recent years, airlines and pilots have speculated that current regulations restricting pilot movement in the cockpit during a flight may influence the frequency of kidney stone formation, though this has not yet been validated or invalidated. This survey is a part of research which will culminate in a presentation and academic paper in my Carmel High School AP Research class. My research should be used to address possible directions that may be taken to remediate the issue of kidney stones in airline pilots.
In attempting to establish kidney stone rates in pilots and possible reasons for the kidney stone development, I ask that you participate in a short survey. This survey will be absolutely anonymous and for use only by the researcher: all recorded responses will be password protected and digitally discarded after this research is concluded. This survey is not mandatory and you will not be penalized for not answering any questions, but any responses will be used to gather data in my project to shed light on the health implications of cockpit safety rules which are currently enforced. If you are interested in participating, please proceed to the above form.

Again, participation is optional, but would be greatly appreciated in the effort to understand the current health situation of airline pilots.

Pilots don't get kidney stones because they can't move around. Pilots get kidney stones because they don't drink enough water. That it's dry at altitude doesn't help.
