As you are doubtless aware, three years ago the FAA was heavily censured by ICHPO, the International Civil Heights for Pilots Organization at the triannual symposium in Zurich. Since that cold day in March, the FAA has tried desperately to regain its international standing by ruthlessly cracking down on medical certificate height discrepancies. It was little realized until the enforcement process had been underway for six months and the statistics began to mount, how staggering the problem was. The FAA turned from its long held efforts at administrative fixes and began seeking civil penalties, and then criminal action in an attempt to stem the tide of rampant height violations. In the last six months, people have simply begin to disappear, without memory or explanation, only to be found days or weeks later. Shorter.

If it's anything like replacing a destroyed medical certificate (OK, the canoe overturned, it happens), the AME isn't ALLOWED to anymore. It'll have to be done by your local regional FAA flight surgeon.
