i had actually talked with my dr about a fusion. he didn't seem to think it would be a problem with the feds. does your dr say different? also, do you have access to alpa aeromedical in denver? they fixed an abnormal ekg for me with the feds (nothing wrong, my heart's just does weird electrical things).
re: the sciatic. yeah, that's EXACTLY the same problem i'm having now. good luck. my partial didn't fix my problem either.

I'm actually calling then this morning, they open 08:30 mtn time. I'll also try AOPA. My doc (Neuro) is not aviation, so she is unaware of the issues at hand. She has advised me to contact ALPA, AOPA or the FAA to clarify before we fuse.

14 A history of intervertebral disc surgery is not disqualifying. If the applicant is asymptomatic, has completely recovered from surgery, is taking no medication, and has suffered no neurological deficit, the Examiner should confirm these facts in a brief statement in Item 60. The Examiner may then issue any class of medical certificate, providing that the individual meets all the medical standards for that class.
