Paperwork for flight physical through VA
 

I've recently got out of the Army and has couple of years of medical with the Veteran Affairs. I'm trying to research what an AME is required to fill out for a 3rd class medical because my VA health physician had completed some several years back and haven't been keeping up with the changes. Trying to keep it in the VA because all my health record is through them and don't want to do more work going back and forth with civilian offices. I tried google and only found that FAA Form 8500-8 is required.

What kind of paperwork is required to complete a 3rd flight physical and where can I get them? Any comment is appreciated

The AME has form 8500-8. You can pull up copies online. First time might be a pain but normally it takes longer to fill in the address block than the 20 yes/no question medical history. They'll want you to list whatever drugs you currently taking and and all your visits to medical professionals and why.

The FAA has an online version MedXpress, you fill out the info on your time and and the AME's office pulls it up when you show. Some AMEs will only do MedXpress because the FAA is starting to require the data be submitted to OKC online and they're outsourcing the receptionist's job to you.

I would also make sure if you do use MedXpress print a copy and bring it because some AMEs still don't use it and they will expect you to fill out an 8500 at their office.

https://medxpress.faa.gov/ fill out online and go the local AME. If you can find a 8500, AME may still ask you to go online and fill out (i would). There may be some VA MDs or VA centers that do this but it's a rarity.

Unless the VA has an approved AME in your area, you're going to be stuck finding a civilian doctor for an FAA physical.
