I’ll say this. I’ve got a failure from PSA. From my experience the reason for failure would have been a debrief item almost anywhere else. I never had trouble with flying the aircraft. Missing a speed on a star and calling for nav instead of hdg on a departure. Wrong? Absolutely, trust me, that has never happened since the LOE. Anywhere else it’s a debrief item. I also know of someone who was hooked for “unapproved products” ie he had notes from previous sims with him. Instead of telling him to put them away the apd hooked him after the check.
