Glad nobody got hurt!....But I have to ask....isn't sombody suposed to be looking out the window and down the runway when the PF pushes the throttles up?

What do your company specs say about starting the roll when there's still another Aircraft on the runway? I know it's happened to me, but I usually wait until they have left it.
