First class medical / 121 app & interview

Have 121 interview, doesn't specify w/I 6 months. Was issued as first class 7 mo. Ago. Perfectly valid for 121 FO. Any reason to believe this could cause heartburn w/ regional interview?

If you have been issued a First Class in the recent past, you should be fine. I assume you're over 40? When I interviewed at Envoy in the past, I had 1st class with 3rd class privileges. I just received a conditional offer stipulating that I get one before they could offer me a class date.

Is there any reason other than cost that you don't want to get it renewed? If not, then just get it done and you are good to go for another 6 mos.
