Yes the flaps were set incorrectly but then PSA then changed the checklist to reduce workload on the taxi out. Was that admission that the taxi check was not safe? Should PSA share some of the blame?

Did I miss anything on fatigue being a factor? They were talking about being tired on the CVR as well. I guess fatigue didn't come into play either? Yeah they screwed the pooch, but any accident report that doesn't examine all the chains in the event, is diminished in value. An interesting comment would be to contrast this overrun with the last CRJ accidents. One commonality: fatigue.

I flew enough with the accident captain and I would say he wasn't normally a chatty cathy, and was a professional. Anyway, they screwed up, broke the rules for sure but fatigue leads to most mistakes in my experience.
