Most handhelds don't go above 10,000 feet. The very latest models get up to 14,000 or so.

News is saying it was a "Buk" missile, which is apparently the Russian name for the SA-11, which is a mobile radar guided missile. If so, it could easily take down a jet flying at 330.

By who?

We have notams and no-fly zones for various regions, applicable to our own aircraft.

MANPADS would be unlikely to drop a widebody anyhow, that's not what they're designed to do.

They're designed as a last-ditch defense for ground troops...basically a TACAIR irritant.

There are routine diplomatic procedures to "warn off" traffic in international water/airspace where combat is occurring.

But this is somebody's airspace. A combatant can of course restrict as much of it's own airspace as it wants to. But the Russians collect overflight fees on many routes...
