Not sure about the first question but it has to be nonstop to and from the scheduled cities.

One minor caveat to this rule occurs in the case of co-terminal airports. If you had a DH say, from EWR-HKG on United (business/first only, no FC), you can use JFK since the NYC airports are considered co-terminals. That would allow you to exercise this option with Cathay, JFK-HKG.
