Documenting SOE for removal of linitation

I'm trying to help a friend get the SOE language removed from his ticket. When I did my first type the airline provided a form for documentation of the 25 hours. It had blanks for date, equipment, time, signatures, etc. I can't find a copy of mine or anything similar online. Would anyone be willing to share something they have used that was accepted by the FSDO or CMO rep? TIA!

Do they fly for a regional? Most regionals will have a internal form to document the info and will work for the FSDO

Not at a 121. He's flying for a 135 but they don't have an internal form. My understanding is that he's one of their only new hires that didn't have an existing type and time.

If there's nothing internal there, then he should make one. Dates, legs, times, signatures, employee numbers should do the trick. He should only log flights that he is PF.

I just did this. We have an internal form but it's nothing official looking. Just an excel table for the information people mentioned above and a paragraph at the top explaining what it is. Corroboration with a company or personal logbook should work. Heck, a plain old logbook with signatures should work. As long as it's official looking enough and the FSDO is familiar with the company which could help.

He/she could also just carry their logbook and have their Captains sign off each flight day.

Which 135? If it’s the one I work for I may be able to provide another idea.
