Had to hand-fly a CRJ-700 (with a deferred A/P) from OKC to SFO nonstop (about 3 hours) at FL280. I was utterly beat by the time we landed. I can't imagine how much more fatiguing it would have been to maintain altitude if we'd been allowed to go even higher. Sure, you can hand-fly all day long...but why would you want to?
