As a result of the first failure in 2018, in 2019 the FAA directed P&W to inspect all PW4000 fan blades using a newer inspection process. I've not read anything yet that said this motor had been reinspected.

But if you listen to the audio tape, the pilot said they never ran the QRH because of the situation at hand
