With that said even if the boxology doesn't dictate that you flag it in the app, might be best to mention it in the interview just to pro-actively defuse the issue. Tell them SUCCINCTLY what and why it happened, what you learned so it won't happen again, then zip it and let them move on. Normally I wouldn't say to disclose anything you don't have to, but they're going to see it in PRIA anyway

IIRC that would not be a training failure, but I haven't read the legaleese on airline apps recently.

agree. I would at least verbally fully disclose in the interview. Or put it somewhere in comments/explanations on app. "Indeed, I was rated satisfactory, but in an abundance of forthrightness, please be aware that XXXXXXX"

my uneducated opinion

(how can HR give you negative points for that?)

They pull your app when they send out the Hogan?
found some hour discrepancy’s from my part 141 college days. Listed too many hours on some planes and not enough on the others. I understand I can correct when I show up for the interview....is this a huge deal ? Definitely more attention to detail needed on my part!

I remember them telling us not to correct hours if it was less than a certain amount (I think it was less than 100hrs) at the interview.

Agreed with above post. If I remember right, we were told corrections within 100 hours isn’t required. What they don’t want to see is undisclosed (prior to then pulling your app off airlineapps) checkride failures, speeding tickets, felonies, DUI, etc etc.

Back when Dorris was still in the hiring department, I asked her about the same thing. How many hours would be enough to warrant a “red pen change” to the printed app? (3 months between the time UAL printed my app to actually showing up in person for a 121 RJ guy can be close to 300 hours (200ish for me)

It was stated that I should make it as close to within 100 hours as possible. So my app had quite a few updates in red...

I would not get too carried away or lose sleep over general aviation numbers. I don't think I corrected my hours even though I had around 150 additional hrs of 121 SIC. They are looking for guys putting down their next type rating 6 months before they go to training or saying they were a director of flight ops when they weren't. Easy little boxes to accidentally check with not good consequences. Good luck!
